FAQ sur les services pour assuré(e)

Q : Victime d’un accident de travail, ou d’une maladie professionnelle comment constituer le dossier ?

R : Le dossier de demande de prise en charge d’un accident de travail ou d’une maladie professionnelle comprend :
- demande d’accident de travail ou de maladie professionnelle bien renseignée et signée par l’employeur ;
- certificat médical initial (et de prolongation le cas échéant) ;
- certificat médical final tous signés par le médecin traitant ;
- des factures et ordonnances le cas échéant ;
- demande de remboursement des frais médicaux et pharmaceutiques ;
- bulletins de paie des 3 mois précédant le sinistre ;
- copie de la CNI ; être à jour dans la télé déclaration pour la période de référence.

Dans le cas d’un accident de trajet (circulation) ou tout autre accident grave (électrocution…) 01
Procès-verbal de constat d’accident (rapport d’enquête ou de gendarmerie).
